Kat Von D and High Voltage Tattoo in conjunction with Goldenvoice are excited to announce the 1 st Annual MusInk Tattoo and Music Festival which will be held February 22, 23 & 24 at the OC Expo Grounds in Costa Mesa, CA. The 2008 show will present to you over 200 artist booth's representing some of the finest tattooers from around the world. There will also be lifestyle vendors, contests, sponsor booths, art exhibits and seminars.

INSURANCE

Standard Shop or Artist Insurance Requirments for MusInk

ARTIST or SHOP: _______________________________________________ As stated on your Artist Booth contract, please have your insurance company forward a certificate of insurance to Goldenvoice as soon as possible. For the convenience of both you and your agent, here is a list of our company's requirements.
Attention is called to the necessity for insurance covering all risks (including but not limited to liability, fire, theft, damage, etc.) on your exhibit from the place to the exhibition facility and return, including the period which the materials remain at the MusInk Tattoo & Music Festival. Vendor acknowledges the management, the Sponsors, and their officers, agents or employees do not maintain insurance maintaining Vendor's property and that is the sole responsibility of the Vendor to obtain insurance for business interruptions, property damage, and other insurance covering such losses as Vendor may incur, and assume no responsibility for the safety of the properties of the Vendor's officers, agents, or employees from theft, damage by fire, accident of any other causes whatsoever. Vendor expressly agrees to save and hold harmless Musink Tattoo & Music Festival, c/o Goldenvoice LLC, AEG, its Management, Sponsors, their officers, its agents, employees and attendees, person and or property in connection with Vendor's use and occupancy of the exhibit space or participation at MusInk.

Vendor agrees to provide Commercial General Liability insurance and further agrees to include as part of its general liability insurance appropriate auto coverage (whether owned, hired or non-owned). Vendor's liability shall be in the amount of $1,000,000 combined single limit for bodily injury and property damage. Such insurance shall be in full force and effect at all times during the Vendor's participation at the Musink Tattoo & Music Festival. Musink Tattoo & Music Festival c/o Goldenvoice, LLC & Goldenvoice Presents, AEG, OC Expo Grounds, its directors, officers, employees, and agents shall be listed as Additional Insured under such insurance and this shall be indicated on the pertinent certificate of insurance.

Certificates of Insurance relating to the coverage listed above shall be furnished by the Vendor at least Fifteen (15) days prior to the Musink Tattoo & Music Festival scheduled for February 22, 23, 24, 2008 by fax at 323-930-7164 or mail to: Goldenvoice, LLC
